Understanding Heart Diagnostic Tests

ECG: Assessing Electrical Activity


An Electrocardiogram (ECG) assesses the electrical status of the heart. It can detect markers of sudden death, such as high-grade arrhythmias (abnormal heart rhythms), long QT syndrome, and Brugada syndrome. Additionally, it identifies patients who may need a permanent pacemaker, especially those with sick sinus syndrome or third-degree atrioventricular block if symptomatic. Furthermore, an ECG can detect acute myocardial infarction (heart attack) or ischemia, which occurs when the heart muscle is deprived of oxygen due to stenosis or obstruction of coronary arteries.


24-48 Hour Holter Monitoring


A standard 5-minute recording of an ECG may not be sufficient to capture heart rhythm abnormalities or ischemic episodes. Holter monitoring, which records the heart's activity over 24 to 48 hours, provides a more comprehensive view of the heart's rhythm. This extended monitoring can help identify issues that might not be apparent during a brief ECG.


Treadmill Stress Test: Unmasking Hidden Issues


Abnormalities in the ECG may not be detected at rest but can be revealed through exercise. The treadmill stress test induces stress on the heart, allowing healthcare providers to observe how the heart responds under physical exertion. This test is essential for diagnosing conditions that may only present symptoms during activity.


Ambulatory Blood Pressure Monitoring


Blood pressure readings taken during a clinic visit may not accurately reflect a patient's true blood pressure status. Some individuals experience "white coat hypertension," where their blood pressure is elevated in a clinical setting but normal at home. Others may have nocturnal hypertension, characterized by elevated blood pressure at night. Ambulatory blood pressure monitoring over 24 hours can identify these conditions, helping to prevent potential heart complications.


2D Echo Doppler Study: Assessing Heart Structure and Function


A 2D Echo Doppler study uses ultrasound technology to evaluate the heart's structure and function. This test can reliably detect heart enlargement and signs of heart failure, particularly in patients with low ejection fraction (HFrEF) or normal ejection fraction but high filling pressure (HFpEF). It is also invaluable for diagnosing congenital heart disease and valvular heart disease, which may require surgical intervention. Segmental wall motion abnormalities observed during this study can suggest coronary artery disease.


Carotid Duplex Scan: Preventing Stroke


The carotid arteries are large vessels that supply blood and oxygen to the brain. Atherosclerosis, or the buildup of fat in the walls of these arteries, can lead to stenosis or obstruction, compromising oxygen supply to the brain and increasing the risk of stroke. Early detection of carotid artery disease through a carotid duplex scan can help prevent serious complications.


Arterial Duplex Scan of Extremities


An arterial duplex scan of the upper and lower extremities can detect peripheral arterial occlusive disease. If left untreated, this condition may lead to gangrene and amputation, particularly in diabetic patients. Early diagnosis is crucial for effective management and treatment.


Venous Duplex Scan: Identifying DVT


A venous duplex scan is essential for detecting deep venous thrombosis (DVT), which can lead to fatal pulmonary embolism. This test can also diagnose venous insufficiency caused by varicose veins, a common cause of bipedal edema, especially in the absence of heart failure, kidney failure, or liver cirrhosis.
